Job Description:

Verifies and enters medication orders, proactively communicating with medical staff to resolve problems. Provide direct patient care, including pharmacotherapy assessment, therapeutic monitoring, pharmacokinetic dosing, and transitions-of-care. Participates in clinical research and trials related to specialized areas. Provides accurate, safe, and timely medication therapy, and promotes cost-effective drug therapy. Precept, mentor, and educate pharmacy students, residents, and staff, and provide continuing education to interdisciplinary team members. Contribute to scholarly activity and professional advancement through publications, presentations, and committee involvement. Other duties as assigned. Leads and facilitates pharmacy practice programs, direct patient care services, and medication systems. Initiates and facilitates appropriate drug dosing and manages medication-related pharmacist consults. Performs all pharmacist functions, actively participating in established standard work parameters and providing coverage of all pharmacy areas. Supervises technician and intern practice, prioritizes and organizes workflow, and checks accuracy of pharmacy technician activities. Accurately manages medication inventories, and prepares and dispenses patient-specific medication doses. Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems.

Knowledge, Skills, and Abilities:

  • N/A

Education:

  • Bachelor's (Required)
  • Doctorate (Preferred)

Field of Study:

  • in Pharmacy

Work Experience:

  • Completion of Post Graduate Year One (PGY1) residency (Required) AND
  • Post Graduate Year Two (PGY2) residency in specialty practice, OR
  • 5+ years of contemporary hospital or specialty practice experience consistent with institutional needs (Required)

Licenses and Certifications:

  • Licensed Pharmacist (RPH) (Required)
  • Basic Life Support - CPR Cert (BLS) (Preferred)

$48.36 - $89.96
