Hours: 40 hours per week; Monday - Friday, 8:00 am to 4:30 pm (no on-call, evenings, or weekends)
Pay Rate: $28.01 to $29.48 per hour
Education: Bachelor’s degree in Public Health, Social Work, Psychology, Human Services or related field with 2 years of experience working in social services or related setting. OR Master’s degree in Public Health, Social Work, Psychology, Human Services or related field with 1 year of experience working in social services or related setting.

Responsibilities
- Provide mobile outreach and engagement
- Assist leading care management and case conference discussion
- Screen and assess for program eligibility
- Promote positive health outcomes for clients utilizing mobile case management, care coordination with other providers, and supportive services with a team of dedicated professionals
- Connect clients and their families to a medical clinic and providers, as well as other clinic and community‑based services
- Arrange transportation and accompany clients to office visits as needed
- Serve as an advocate for the client and family
- Use motivational interviewing, trauma‑informed care, and harm‑reduction practices
- Create and maintain all required documents in the electronic and paper record systems
Qualifications and Skills
- At least six months of experience working with youth strongly preferred
- Experience working with Seriously Mentally Ill (SMI) population is preferred
- A valid California driver’s license in good standing, personal vehicle, proof of insurance and eligibility to be a CRF Approved Driver are required to fulfill the essential functions of this position
- Ability to pass criminal/background clearance
