LCSW Social Work Care Manager

Kaiser Permanente seeks an LCSW Social Work Care Manager within our Healthcare & Medical Services department. In this role, you’ll provide biopsychosocial assessments, care planning, brief therapy, and care coordination for diverse patient populations. You will collaborate closely with physicians, nurses, and interdisciplinary teams to address social determinants of health, support chronic disease management, and connect patients to community resources.
