About the Role
We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our integrated care team. In this role, you will provide supportive counseling, crisis intervention, and medical social services through comprehensive psychosocial assessments and collaborative treatment planning. Your work will make a meaningful difference in the lives of individuals and families navigating medical, mental health, and social challenges.
Key Responsibilities
  • Provide individual and group counseling, both virtually and in person.
  • Conduct biopsychosocial assessments, treatment plans, and progress notes.
  • Collaborate with inter-professional care teams to coordinate patient care.
  • Deliver crisis intervention and warm hand-offs as needed.
  • Refer patients to appropriate community services and advocate for access.
  • Maintain accurate, timely documentation and adhere to NASW Code of Ethics.
  • Participate in performance improvement initiatives and community outreach.
  • Maintain licensure, CEUs, and uphold professional standards.
Qualifications
Minimum Requirements:
  • Master's Degree in Social Work (MSW)
  • Valid Florida Driver's License
Preferred Experience:
  • Experience working in a Federally Qualified Health Center (FQHC)
  • Familiarity with Electronic Medical Records (EMR)
Skills & Attributes
  • Excellent clinical, interpersonal, and analytical skills
  • Strong verbal and written communication abilities
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint)
  • High level of professionalism, integrity, and confidentiality
  • Commitment to equity, community service, and holistic care
  • Ability to manage a diverse workload in a fast-paced environment
Work Environment & Physical Requirements
  • Office-based and clinical setting
  • Regular use of standard office equipment
  • Must be able to walk, stand, talk, hear, and use hands to operate equipment
