Base pay range

$45.00/hr - $50.00/hr

Duties

• Conduct biopsychosocial assessments to collect and analyze patient information.
• Develop and implement plans of care to meet patient and caregiver needs.
• Collaborate with a multi-disciplinary team to ensure continuity of care.
• Educate patients and caregivers to facilitate discharge and promote understanding of care plans.
• Provide patient and family education, support, and advocacy.
• Facilitate discharge and contingency planning.
• Document patient care, including delivery, plans, goals, and patient responses, according to plan of care.

Skills

• Biopsychosocial assessment
• Provide patient and family education, support, and advocacy
• Work with a multi-disciplinary team to ensure continuity of care
• Knowledge of psychiatric hospital settings
• Understanding of short-term therapy utilization
• Knowledge of protective services, such as adult and child protective services
• Management/lead experience
• Familiarity with community programs, including Medicaid, Medicare, and social services

Experience

• Minimum of 2 years of experience in a relevant field.
• Experience with the EPIC charting system is preferred.
• Community hospital experience is preferred.
