To support transitions of care for members in Kentucky, the full-time Kentucky Licensed RN Care Manager will develop and facilitate personalized care plans, assess member needs, and coordinate appropriate services while working remotely from Monday to Friday, 8 AM to 5 PM CT or ET. Key responsibilities Evaluates member needs and barriers to care, recommending plans for optimal health outcomes Develops ongoing care plans and collaborates with providers to address members' unmet needs Provides education to members and their families on disease processes and healthcare options Required qualifications Active Kentucky State or Compact RN license Degree from an Accredited School of Nursing or Bachelor's degree in Nursing 2-4 years of related experience in medical-surgical, inpatient, or outpatient settings Experience in care management or home health preferred Knowledge of social determinants of health and community resources
