We foster a culture of innovation while creating environments that allow us to focus on what matters most—providing healthcare and wellness services that make a real impact.Position SummaryThe Juvenile Justice Case Manager provides assessment, counseling, and service coordination for court-involved youth at risk of recidivism. This role focuses on case management, group facilitation, advocacy, life skills development, and educational programming to support positive outcomes.Key ResponsibilitiesManage an active caseload of 5–10 clientsComplete intake and enter client data into YASI within 72 hoursDevelop and maintain individualized case plansFacilitate individual and group sessions using evidence-based curriculumUse motivational interviewing and de-escalation techniquesCollaborate with probation, law enforcement, schools, families, and providersCommunicate client needs to Program Coordinator and clinical teamEnsure client safety during all programmingTransport clients and support program logistics (including food delivery)Maintain agency vehicles as neededComplete all documentation and incident reports accurately and on timeParticipate in required trainings and maintain Food Handler CertificationWork ScheduleFlexible based on program needsEvenings and weekends requiredWork across multiple reporting center locationsCare Coordination May support or participate in care coordination efforts within the program and assigned geographic area.Trauma-Informed Approach All work is guided by trauma-informed care principles:SafetySelf-regulationSupportivenessStrengths-based approachSelf-careCompensation & BenefitsHealth, Dental, and Vision InsuranceDisability and Life InsurancePaid Time Off (Vacation, Sick, Holidays)Retirement PlanEmployee Assistance Program (EAP)Employee DiscountsQualificationsBachelor's degree in Human Services or related field requiredExperience working with at-risk or justice-involved youth preferredStrong communication and organizational skillsAbility to manage multiple priorities in a fast-paced environment
