As one of the nation's largest nonprofit Catholic healthcare organizations, CommonSpirit Health delivers more than 20 million patient encounters annually through more than 2,300 clinics, care sites and 158 hospital-based locations, in addition to its home-based services and virtual care offerings.As our Job Coach, you will support people with developmental disabilities to have a full quality of life.Every day you will assist people supported to gain skills to be more independent in all aspects of their life, achieve goals and dreams, remain active in the community, gain or maintain natural supports, maintain health and wellness, and be an advocate for each person you support to ensure his/her wants and needs are put first.To be successful in this role, you will effectively empower individuals with developmental disabilities to achieve greater independence and an enhanced quality of life, acting as a dedicated advocate and facilitator for their personal growth and well-being.LEARNING ABOUT THE PERSON SUPPORTEDBuilds a positive rapport with people by learning their wants, needs, and desiresLearns about the person's spirituality, social history and life experiences through education and a variety of community experiencesUses the person's plan, Personal Outcome Measures information, and other assessments to learn about each person's goals and dreams. Supports each person to direct their own services, achieve their goals and dreams, and remove barriers that hinder the accomplishment of the person's goals and dreamsSPECIFIC JOB RESPONSIBILITIESEnsures each person within the department has all of his/her basic assurances metAttends program training with the QIDP and accurately implements all OSP/PCSP programs, learning objectives, and supports.

Attends team meetings and completes assessments as requestedDocuments in an accurate and timely manner in all applicable records, charts and files of people supportedINTEGRATION AND COMMUNITY SUPPORTLearns about people's vocational, community and social interests through experience and exposureAssists people to remain connected to their natural supports (family, close friends, boyfriend, girlfriend, etc.)Supports people to be active members of the community (i.e., employment, volunteer work, belonging to clubs/organizations, participating in church activities, etc.) by emphasizing choices and optionsMaintains a valid driver's license, vehicle insurance, and has access to a vehicleHEALTH, SAFETY AND WELFARE OF THE PERSON SUPPORTEDLearns about the best possible health needs (including physical and mental health) for the person you are supporting, including knowledge of medications taken by the person and the effects of the medicationsSupports people to become involved in their personal health needs, accompanies people to medical appointments, provides support during medical procedures, and completes follow-up recommendationsAdministers medications and completes treatments according to the Medication Administration RecordMaintains current First Aid and CPR certificationAssists and supports people to have a safe and clean environment in which to live and workLearns about nutritional and healthy lifestyle recommendations through meal planning and preparationSupports people to look their best by providing assistance in areas of hygiene, grooming, and attireFINANCIAL MANAGEMENT OF DEPARTMENTAssists people supported to learn about his/her personal finances including money management and budgetingAssists with financial expenditures/budgets for people supported. This includes cash, checks, debit cards, and EBT cards. Assists people supported to pay their bills and keep an accurate record on all financial spending ledgers.Uses department's procurement (P) card appropriatelySYSTEM ADVOCACYAssists and supports people to learn about their rights, due process, and fair treatmentEnsures people we support are free of abuse, neglect, mistreatment, and exploitationServe as a personal advocate for people and teach members of the community to treat people we support with dignity and respect through respectful communication and interactionsAdvocate and job source with employers, community members, families and other stakeholders to develop needed supports and servicesJob RequirementsRequiredHigh School or equivalent diploma.
