Lutheran Social Services seeks a Bilingual Spanish IRIS Consultant to support adults with long-term care needs in self-directing their services. In this community-based role, you will conduct in-home and virtual visits, assess needs and goals, and develop individualized IRIS support plans that promote independence and community inclusion. You will educate participants and families on options and resources, coordinate services, and advocate for their preferences while maintaining accurate documentation.
