Iowa/South Dakota Licensed Case Manager

To support members with chronic and complex healthcare needs, the remote-eligible Iowa/South Dakota Licensed Case Manager will create and manage individualized care plans, coordinate services with healthcare providers, and actively engage with members and their families. Key responsibilities Utilize critical thinking to identify high-risk conditions and anticipate member needs within the healthcare system Design and manage individualized care plans while coordinating with members, families, and healthcare providers Document care management activities accurately and comply with regulatory and accreditation standards Required qualifications Completion of an accredited nursing program Active and unrestricted RN License in Iowa or South Dakota 4+ years of diverse clinical experience in settings such as acute care or home health Must obtain Certified Case Manager (CCM) certification within 24 months of hire Experience in care navigation or care coordination is strongly preferred
