Key responsibilities: Evaluate member needs and barriers to care, recommending personalized care plans for optimal outcomes Develop ongoing service plans and collaborate with providers to address unmet needs Monitor member progress towards care plan goals and provide education to members and their families on healthcare options Required qualifications: Active Iowa RN license or Compact State Licensure Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing 2-4 years of related experience in case management and maternal health Experience with prenatal or post-partum care is preferred Familiarity with OB or NICU care is a bonus
