Iowa Licensed Case Management Nurse

Dedicated to supporting members' chronic and complex health care needs, the remote-eligible Iowa Licensed Case Management Nurse will create and manage individualized care plans, coordinate services with health care providers, and foster strong relationships with members and their families. Key responsibilities Utilize critical thinking to assess and identify high-risk conditions, proactively anticipating member needs in navigating the health care system Design and manage individualized care plans while coordinating communication between members, providers, and community resources Document care management activities accurately and in compliance with regulatory standards, ensuring quality and consistency in care delivery Required qualifications Completion of an accredited nursing program Active and unrestricted RN License in Iowa or South Dakota Certified Case Manager (CCM); must obtain within 24 months of hire 4+ years of diverse clinical experience in settings such as acute care, outpatient, or home health Strong technical acumen with proficiency in Microsoft Office and clinical documentation platforms
