- Develop and implement individualized care plans
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
- Offer emotional support and counseling to patients and families
- Document patient progress and maintain accurate records
- Support patients through end-of-life processes with compassion and professionalism
- Valid state licensure as a Social Worker or relevant certification
- Experience in hospice or palliative care preferred
- Strong communication and compassionate bedside manner
- Ability to work independently and as part of a team
- Key skills: empathy, organizational skills, cultural sensitivity
