Responsibilities
- Provide outpatient primary care and chronic disease management
- Perform history & physicals, admissions, evaluations, and treatment plans
- Order and interpret diagnostic tests
- Coordinate care with specialists and multidisciplinary teams
- Participate in acute care stabilization and code response when needed
- Complete documentation in VA CPRS/EMR
- MD or DO
- Board Certified/Eligible in Internal Medicine or Family Medicine
- Active unrestricted Wisconsin or Minnesota medical license
- Current ACLS & BLS certifications
- DEA and NPI required
- Completion of accredited residency program
