Internal Medicine Physician

Location: Stockton, CA

Schedule: Monday - Friday, Day Shift
Setting: Outpatient

• Provide comprehensive primary care services, including assessments, diagnosis, treatment, and chronic disease management for adult and geriatric patients.

• Conduct physical examinations, review medical histories, order diagnostic tests, and develop individualized treatment plans.

• Collaborate with the interdisciplinary team (IDT) to coordinate and monitor participant care and overall health outcomes.

• Prescribe and manage medications, preventive care, and health promotion programs for patients.

• Educate patients and families on health maintenance, chronic conditions, and treatment plans.

• Supervise and collaborate with Nurse Practitioners and Physician Assistants while coordinating specialist referrals as needed.

• Maintain accurate medical documentation, participate in quality improvement initiatives, and ensure compliance with organizational policies and healthcare regulations.

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.

• Active California medical license required.

• Current DEA registration required.

• Board Certified in Internal Medicine or Family Medicine is required.

• Minimum of 1 year of experience working with frail or elderly populations.

• Geriatric care experience or added qualifications in geriatrics required.
