This full-time, clinic-only role operates Monday through Friday, 8am to 5pm, with no call responsibilities. The successful candidate will focus on helping patients manage severe long-term illnesses and multiple, complex chronic conditions within a primary care setting.
Practice Info
- Outpatient (Clinic Only)
- Patients per day: 8-12
- Staffing Mix: 3 APPs in clinic, supported by nursing staff and MAs

Responsibilities
- Manage severe long-term illnesses and multiple, complex chronic conditions.
- Provide primary care services, addressing conditions such as heart disease, hypertension, type 2 diabetes, COPD, tick-borne illnesses like Lyme disease, and age-related multi-morbidity.
