Client is seeking Internal Medicine or Family Medicine Hospitalists to join a large, collaborative Hospital Medicine program in Clovis, California.
This is a high-volume practice with an average daily census of approximately 284 patients and 40 admissions per day. The Hospital Medicine team includes dedicated rounders, admitting physicians, swing physicians, nocturnists, and APP support.
The practice emphasizes strong communication with patients and families, close collaboration with specialty services, and comprehensive management of medically complex patients. Hospitalists should be comfortable managing patients who may require a higher level of care than is typical in many community hospitals.

Hospitalists manage step-down patients and medically complex patients who, in other hospitals, might otherwise be managed in an ICU.

Daily census and responsibilities may vary based on seasonality
Day Admitter:

1 admitting physician
Responsible for admissions and related medical responsibilities
Swing Hospitalists:
Swing 1: 1:00 PM 1:00 AM
Swing 2: 3:00 PM 3:00 AM
Swing 3: 3:00 PM 3:00 AM
Swing shift times may be adjusted based on admission-pattern data.
Nocturnists:
7:00 PM 7:00 AM
2 nocturnists
Approximately 6 7 admissions per nocturnist
Cross-cover approximately patients
Respond to rapid responses
Nocturnist APP support may be incorporated into the evolving staffing model
Clinical Responsibilities:
Hospitalists will be responsible for:
Comprehensive inpatient medical management.
Daily rounding and treatment planning.
Admission evaluation and management.
Cross-coverage of hospitalized patients.
Management of medically complex and step-down patients.
Supervision of APPs.
Responding to rapid response events involving their patients.
Close collaboration with consulting specialists.
Frequent communication with patients and families regarding treatment plans and progress.
The admitting team is responsible for:
Completing admissions within one hour of being notified.
Responding to rapid responses.
Transitioning patients from the ICU to the medical floor.
Completing medical consultations for patients admitted under specialty services.
Codes & Rapid Responses:
The ED team manages codes occurring in the Emergency Department.
Critical care/code teams manage codes outside the ED.
Hospitalists respond to rapid responses involving their patients.
The daytime admitting team has primary responsibility for rapid responses.
Procedures:
No specific invasive procedures are required for the Hospital Medicine position.
Hospitalists should, however, be comfortable caring for medically complex patients, including those requiring significant respiratory support.
Patient & Family Engagement:
Patient and family involvement is a major component of the practice.
Hospitalists should expect:
Frequent family meetings.
Regular discussions regarding treatment plans and clinical progress.
Strong emphasis on clear, compassionate communication.
Active participation from patients and families in care decisions.
Excellent communication skills and a patient-centered approach are essential.

Board Status:
Board Certified physicians preferred/required for established clinicians.
Board Eligible physicians may be considered according to applicable specialty-board and hospital bylaws.
Board Eligibility is generally accepted for up to 5 years following completion of residency, subject to applicable requirements.
Required Certification:
ACLS
Current DEA certificate
Current state medical license required on file before the MSO can begin the provider's work process. The license does not necessarily need to be a California license at the initial stage.
Credentialing:
Typical credentialing timeframe: days
During busy periods or system go-live periods, credentialing may take up to 120 days
Temporary privileges may be available when patient-care needs require them
Temporary privileges are available only after the credentialing file is complete and ready for review
No red flags are permitted for temporary privileges
No fees are required for temporary privileges
Clinical Documentation Requirement:
Credentialing requires recent documentation demonstrating experience managing:
Patients on mechanical ventilation for less than 24 hours, and
Stable patients requiring long-term/chronic ventilation
Relevant logs must cover the previous 24 months.

Applicants should be prepared to provide:
Current CV in MM/YY format with no unexplained gaps
Current DEA certificate
ACLS and other applicable certifications
Passport-style color photograph
Driver's license
Social Security card or valid U.S. passport
Documentation supporting requested privileges
Procedure and clinical activity logs covering the previous 24 months
TB screening completed within the previous 12 months
COVID-19 vaccination/booster documentation, if applicable
Current influenza vaccination documentation
