A mission-driven community health organization in Austin, Texas , is seeking an experienced Adult Medicine Nurse Practitioner to join its integrated primary care team.

Responsibilities
- Provide comprehensive outpatient primary care to adult patients
- Conduct patient histories, physical examinations and clinical assessments
- Diagnose and treat acute and chronic medical conditions
- Develop individualized, evidence-based treatment plans
- Manage medications and chronic disease care
- Order and interpret laboratory studies and diagnostic testing
- Provide preventive screenings and routine health-maintenance services
- Educate patients regarding treatment, prevention and self-management
- Coordinate referrals to specialists and community resources
- Collaborate with physicians, Medical Assistants, behavioral health professionals, nutrition staff and other care-team members
- Maintain complete, accurate and timely clinical documentation
- Support continuity of care for medically and socially complex patients
- Participate in interdisciplinary care planning and quality-improvement efforts

- Substance use disorder treatment
