An innovative, integrated primary care organization is seeking a Board-Certified Family Medicine or Internal Medicine Physician (MD/DO) to join its practice in Old Fourth Ward, Atlanta.
This opportunity is designed for physicians who value whole-person care and meaningful, long-term patient relationships. You will work alongside behavioral health providers, nutrition professionals, advanced practice providers (APPs), nurses, and care coordinators to address patients medical, behavioral, nutritional, and preventive health needs.
The role combines comprehensive outpatient medicine with clinical leadership, interdisciplinary collaboration, and opportunities to improve care delivery.
As a Primary Care Physician, you will provide comprehensive outpatient care while serving as the clinical leader for your patient panel.
You will evaluate acute concerns, manage chronic conditions, provide preventive services, and develop individualized treatment plans based on each patient's medical history, clinical needs, preferences, and health goals.
You will also collaborate with the broader care team to connect patients with behavioral health, nutrition, and care coordination resources. Through consultation and mentorship, you will support APP colleagues and help promote consistent clinical standards throughout the practice.
