• Hospital-employed position
• Focus on value-based care for complex geriatric patients aged 65 and older
• Manageable patient census of 10 - 14 patients per day
• Shared call among all providers; RN call center handles routine calls

Shift & Schedule

• Monday - Friday schedule, 8 am - 5 pm with minimal triage call

Requirements

• Board-certified or board-eligible
• DEA required
• Experience with HEDIS measures and HCC coding highly valued
• eClinicalWorks EMR experience preferred
