Michael Hill, MD and Associates

Interim Chief Financial Officer

Remote Part Time Hospitals and Health Care

Company:  Michael Hill, MD and Associates (MHMDAA)

Reports to:  Michael Hill, MD — President & Chief Executive Officer, Chief of Staff & Operations

Engagement:  Independent contractor (1099), approximately 20–30 hours per week

Location:  Fully remote, United States — core availability during Eastern business hours

Why This Role Exists

Michael Hill, MD and Associates has grown from a founder-led expert witness practice into a claims-review firm running multi-hundred-claim matters for national law firms and their payer clients. The clinical and analytical side of the business has scaled fast. The financial side now needs to catch up.

We are looking for an interim, part-time CFO to bring executive-level financial discipline to a distributed contractor business with an unusually long and complex revenue cycle — and to build the reporting, controls, and cash visibility a firm of our size and trajectory should already have.

About Us

MHMDAA provides expert witness opinions, clinical claims review, and coding and utilization analysis in payer–provider disputes. Our clients are national law firms — and, through them, the health plans and providers they represent. Our work product is the expert report that decides whether a disputed claim, or several hundred of them, was properly adjudicated.

We are roughly three years old, fully remote, and run on a network of about 30–35 independent contractors: nurse reviewers, physicians, certified coders, and a technical and business-development team based in India. There is no office. Everything runs on Microsoft 365, SharePoint, Teams, Clockify, and QuickBooks, with an increasing amount of the review workflow supported by AI tooling.

What You’ll Own

Cash and the revenue cycle

  • Own cash forecasting on a rolling 13-week basis, including the timing mismatch created by our revenue cycle: we invoice the law firm, the law firm bills the payer, the payer pays the firm, and the firm pays us — typically 60 to 75+ days end to end, while contractor obligations accrue continuously.
  • Own accounts receivable end to end — aging, escalation cadence, and direct, professional follow-up with law firm billing contacts and engagement partners.
  • Build a defensible contractor payment schedule that reflects real collection timing, and communicate it clearly enough that the field stops guessing.

Billing integrity and client-facing invoices

  • Own the monthly invoicing cycle: timesheet validation in Clockify, hours-and-rates roll-up by contributor and by matter, supporting documentation, and delivery on schedule.
  • Build and run the exception reporting that catches problems before the client does — duplicate and overlapping time entries, entries split across multiple matters, hours outside expected ranges, and time booked to internal or product work that should never reach a client bill.
  • Produce the narrative cover letter that accompanies any invoice with a material change from the prior period, explaining adjustments and reductions in plain language a general counsel can follow.

Accounting operations and controls

  • Own the month-end close, the general ledger, and the accuracy of QuickBooks; produce a monthly financial package with P&L, cash position, AR aging, and matter-level margin.
  • Own accounts payable for a domestic and international contractor base, including 1099, W-9, and W-8BEN compliance and international payments to our India-based team.
  • Establish and document internal controls — segregation of duties, payment-routing and bank-detail change verification, approval thresholds, and access management — and train the team on them.
  • Own vendor, subscription, and software spend: consolidate redundancies, renegotiate where there is leverage, and serve as the single approval point between operations and finance.

Planning and decision support

  • Build matter-level economics: budgeted versus actual hours by project and by individual, effective realized rate, and project profitability that leadership can see while a matter is still running — not after it closes.
  • Support pricing and staffing decisions on new engagements, including the finance review of client contracts and rate structures.
  • Partner with the Chief of Staff & Operations and the PMO to translate operational data — claim volumes, review throughput, contractor capacity — into a financial forecast.
  • Provide direction and quality review to the existing accounting and finance support staff.

What We’re Looking For

Required

  • 10+ years of progressive finance and accounting experience, including time as a CFO, Controller, or Finance Director with full ownership of the close.
  • Direct experience in a professional services, consulting, or legal services environment billing on a time-and-materials basis — you understand timekeeping, WIP, realization, and why a bill gets disputed.
  • Hands-on experience with a long, intermediated collections cycle and the cash management that comes with it.
  • Working proficiency in QuickBooks and advanced Excel; comfort with a time-tracking platform such as Clockify and an AP platform such as Bill.com.
  • Experience paying independent contractors, including 1099 and W-9 administration and international payments with W-8BEN documentation.
  • A track record of building internal controls and process documentation where none existed — not just operating someone else’s.
  • Executive presence and clear written communication: you will be in front of law firm clients and our CEO, often on the same day.
  • Genuine comfort in a small, fast-moving, fully remote company where the scope of the role will change as the firm grows.

Preferred

  • CPA, MBA, or equivalent credential.
  • Healthcare, health plan, revenue cycle, or litigation support industry background.
  • Experience supporting a company through a transition from founder-managed finances to a structured finance function.
  • Experience managing a distributed team across US and India time zones.
  • Fluency with automation or AI tooling applied to finance and audit workflows — we use it, and we intend to use more of it.

Engagement Details

  • Independent contractor (1099) engagement; this is not a W-2 employee position.
  • Approximately 20–30 hours per week, with volume concentrated around month-end close and the first week of the month for invoicing.
  • Fully remote within the United States. Our default operating time zone is Eastern; daily and weekly operating calls are scheduled during Eastern business hours.
  • Interim engagement with the potential to expand in scope or hours as the firm grows.
