Interdisciplinary Case Manager – Flexible Schedule & Growth

PAM Health is seeking a Case Manager to coordinate care across disciplines for a designated patient population within an inpatient setting.

You will review admission records, assess discharge needs, and coordinate discharge plans while working with a collaborative care team to ensure appropriate utilization of hospital resources and favorable outcomes. Knowledge of payor relationships will be beneficial.
