In this role, you will coordinate care plans, link clients to community resources, and facilitate communication among therapists, medical providers, schools, and agencies. You will complete assessments, track outcomes, schedule and follow up on appointments, and ensure services are delivered according to treatment goals.
You’ll collaborate on multidisciplinary teams, maintain accurate documentation, and provide client-centered, trauma-informed support in homes, offices, and community settings.
