To support patient transitions to home care services, the full-time Intake Admissions Specialist will manage daily patient referral operations, communicate with referral sources, and ensure compliance with regulatory requirements while working remotely from Kettering, OH. Key responsibilities Process all daily patient referral and intake operations Coordinate care by working with multiple service providers and obtaining physician orders as needed Follow up with patients regarding Start of Care (SOC) and services provided to measure satisfaction Required qualifications Associates degree or equivalent healthcare/home care experience required Two years of healthcare experience, with one year in home care preferred Experience in Medical Terminology is preferred Knowledge of government regulations and private payer practices Ability to meet all physical demands of the role
