About the Position
Provide high‑touch palliative care for people facing serious illness in a hospital setting at Cooperman Barnabas Medical Center. The role is supported by structured mentorship, physician/APP backup, and a dedicated team.
Responsibilities
- Perform patient‑centered interdisciplinary palliative medicine at the top of your license.
- Consult and follow‑up across ED, ICU, and medical/surgical units.
- Manage complex symptoms for patients with advanced illness and chronic disease.
- Lead goals‑of‑care and advance care planning conversations; coordinate with SNFs and hospices as appropriate.
- Integrate with the interdisciplinary and hospital teams; shape palliative practice workflows.
- Document timely and accurately in line with clinical, billing, and quality standards.
- Balance discharge planning with intake, managing transitions from inpatient to next settings.
Qualifications
- Active New Jersey Nurse Practitioner or Physician Assistant license; national board certification.
- DEA and CDS registration or ability to obtain by start date.
- 2+ years of palliative care experience; palliative care certification (e.g., ACHPN/CHPN) preferred.
- Strong communication, clinical judgment, compassion; comfort in interdisciplinary teamwork.

- Competitive base salary: $130,000–$160,000 per year.
