Compensation & Total Rewards
- Base salary: $130,000–$160,000 per year
- Annual performance bonus up to 12.5% of base salary
- 28 paid time off days in year one, increasing to 33 days after 2 years in a flexible bank

Responsibilities
- Provide high‑touch, patient‑centered, evidence‑based palliative care across the hospital continuum, from high‑acuity ED and ICU encounters to ongoing management on medical, surgical, and specialty units.
- Manage complex symptoms for patients with advanced illness and chronic disease.
- Lead goals‑of‑care and advance care planning conversations with patients and families, coordinating with partner SNFs and/or hospices when appropriate.
- Integrate fully into interdisciplinary and hospital workflows, partnering with physicians, APPs, social work, care coordination, and operational staff to shape the palliative medicine practice.
- Complete timely documentation consistent with clinical, billing, and quality standards.
- Balance discharge planning with intake to manage transitions from inpatient to the patient’s next setting, honoring each patient’s longitudinal preferences and best interests.
Qualifications
- Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.
- DEA and CDS registration, or ability to obtain by start date.
- Minimum 2 years of palliative care experience, with Palliative Care specialty certification (e.g., ACHPN / CHPN) preferred.
- Strong communication, clinical judgment, compassion, and comfort working within interdisciplinary teams.
- Ability to perform at the top of your license with best‑practice, holistic care.
