Inpatient Case Manager – RN

Monday – Friday: 8:00 am – 4:30 pm, rotating weekends and holidays
40 hours per week
Overview
Responsible and accountable for the provision and facilitation of comprehensive case management to ensure quality outcomes for patients across the continuum of care. The focus is on transitions to appropriate levels of care while facilitating clinical appropriateness and length of stay. Directly support in‑hospital patients identified in need of comprehensive case management and collaborate with the health care team and patients/caregivers to manage care transitions and ensure effective utilization of health services.
Key Responsibilities
- Develop and implement comprehensive care plans based on assessment and evaluation, addressing the physical, mental, and psychosocial needs of inpatients and their caregivers.
- Coordinate and monitor the delivery of health services to support clinical goals and performance metrics.
- Facilitate patient transitions to appropriate levels of care, ensuring timely and appropriate discharge planning.
- Advise and support the health care team, patients, and family members regarding care plans and resources.
Qualifications
- Certification in Case Management (CCM, CMAC, or ACM‑SW) required within one year of eligibility.
- Minimum of 1 year of acute care and/or healthcare experience; educational internship in acute care may be considered. Experience in long‑term care, home health, hospice, or SNF is useful.
- BLS certification required within 90 days of hire.
- MLD – Master’s Level Degree in Social Work.
- Strong communication and collaborative skills; proven ability to manage complex patient transitions.
