Overview
Sentara RMH Medical Center is now hiring an Inpatient Case Manager. The role is responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, with primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay.

Experience
- 1 year of Acute Care Nursing experience
- 1 year of Case Management experience
- 1 year of nursing experience
Duties / Responsibilities
- Responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, with primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay.
- Directly support those in‑hospital patients identified in need of comprehensive case management.
- Promote effective utilization and monitoring of health services, collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions.
- Develop and/or implement a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver.
