Inpatient Case Manager - Registered Nurse

Overview

Full‑time RN Case Manager for inpatient units (house‑wide). Sign‑on bonus available for qualified candidates.

Responsibilities

• Provide continuous care to patients and families throughout the inpatient stay.
• Develop and maintain therapeutic relationships with patients, families, and interdisciplinary team members.
• Serve as a clinical leader, guiding the healthcare team and ensuring safe, effective transition plans.
• Collaborate with nurses, social workers, and other professionals to leverage each other’s expertise.
• Support hospital and organizational strategic initiatives focused on continuous improvement of patient care.
• Apply complex critical‑thinking skills to clinical decision making.
• Facilitate comprehensive case management, services, and quality outcomes for patients across the continuum of care, emphasizing transition to the appropriate level of care and optimal length of stay.
• Provide direct support to hospitalized patients identified as needing comprehensive case management.
• Promote effective utilization and monitoring of health services; collaborate and communicate with the healthcare team and patient/caregiver to manage care transitions.
• Develop and implement comprehensive care plans based on assessment and evaluation, addressing physical, mental, and psychosocial needs of the inpatient and caregiver.

Qualifications

• Registered Nurse (RN) with a minimum of 12 months of nursing experience.
