Inpatient Case Manager/ Registered Nurse (RN)

Overview

The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum. The primary focus is patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay. This position directly supports hospital patients who are identified as needing comprehensive case management and promotes effective utilization and monitoring of health services, collaborating with the healthcare team and patient/caregiver to manage care and transitions. The role develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental, and psychosocial needs of the inpatient or caregiver. The practice setting is hospitals only.

Requirements

• 12 months nursing experience


• Registered Nurse


• Basic Life Support (within 90 days)


• BSN-preferred


• Case Management certification (ACM, CCM, CCCTM, CMAC or CMGT-BC) required within one year of eligibility


• 1 year acute care case management preferred
