Overview
Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management and quality outcomes for patients across the healthcare continuum, with a primary focus on patient transition to the appropriate level of care while ensuring clinical appropriateness and length of stay.
Supports hospital patients identified in need of comprehensive case management, promotes effective utilization of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions, and develops/implements a comprehensive care plan based on assessment of physical, mental, and psychosocial needs.

Responsibilities
- Facilitate comprehensive care management services for patients across the healthcare continuum.
- Focus on patient transition to the appropriate level of care and facilitate clinical appropriateness and length of stay.
- Support hospital patients identified in need of comprehensive case management.
- Promote effective utilization and monitoring of health services.
- Collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions.
- Develop and/or implement a comprehensive care plan based on assessment and evaluation of physical, mental, and psychosocial needs.
Requirements
- Minimum 12 months registered nursing experience.
- Registered Nurse license.
- Basic Life Support certification within 90 days of hire.
- BSN preferred.
- Case Management certification (ACM, CCM, CCCTM, CMAC, or CMGT-BC) required within one year of eligibility.
- Preferred: 1 year acute care case management experience.
