Inpatient Case Manager (Onsite/Field)
Northridge, CA
Position Details:
  • Pay : $ /hr (LVN) $45.-50/hr (RN)
  • Schedule : Monday to Friday, 8:30 am - 5:00 pm
  • Location : Hybrid, in-office in Northridge and field visits in the San Fernando Valley

Key Responsibilities:
  • Patient Management : Assess, plan, monitor, and evaluate patient care during acute care episodes
  • Collaboration : Work closely with physicians and healthcare practitioners to develop and implement care plans
  • Communication : Keep patients, families, and healthcare teams informed about treatment plans and discharge procedures
  • Documentation : Ensure accurate and timely recording of patient care plans and follow-ups
  • Discharge Planning : Facilitate safe and efficient transitions from hospital to home or other care facilities
Qualifications:
  • Current California LVN or RN license
  • At least 2 years of recent experience as an Inpatient Case Manager
  • At least 2 years of experience with Utilization Review
  • Previous clinical experience
  • Proficient in MS Office, with strong computer skills
