Inpatient Care Manager RN: Discharge and Care Coordination

Atlantic Health Morristown Medical Center is seeking a Nurse Case Manager to oversee patient care across the illness continuum. You will work with the Case Management leadership to assess, plan, implement, coordinate, monitor and evaluate services to maximize patient health and resource use.

You will lead interdisciplinary rounds, review utilization, enable timely discharges, and ensure thorough clinical documentation in the EMR. Nursing licensure in NJ and acute care experience are essential.
