Role Overview
The Pharmacist ensures the delivery of safe, effective, and cost-efficient medication therapy, focusing on patient outcomes and quality of life. They develop, recommend, and monitor evidence-based pharmacotherapy care plans, assessing medication appropriateness, safety, efficacy, and adherence.
Key Responsibilities
- Provide pharmaceutical care tailored to the specific patient care area.
- Interpret and clarify physicians’ medication orders, review patient medication profiles, and monitor lab results.
- Suggest formulary alternatives and advise medical staff on drug therapy issues.
- Provide drug information to healthcare professionals and conduct discharge medication consultations.
- Make rounds with medical teams, calculate drug dosages, and verify provider orders.
- Prepare compounded and sterile medications, manage supplies, and supervise pharmacy staff, students, and interns.
- Coordinate with healthcare teams to resolve pharmacy-related issues and assist with departmental policies.
Qualifications & Skills
- Bachelor’s Degree in Pharmacy required; Master’s preferred.
- Strong knowledge of pharmacotherapy, medication safety, and clinical procedures.
- Excellent communication, collaboration, and supervisory skills.
- Ability to interpret lab results, perform dose calculations, and manage pharmacy operations.
