To support patient care through effective symptom assessment, the full-time Indiana Licensed Telehealth Triage Nurse will manage phone and video encounters, utilizing evidence-based protocols while working primarily on Thursdays, alternating days, and every other weekend from 8 PM to 8 AM, with the potential for remote work after training. Key responsibilities Accept and triage patient calls, determining appropriate care and next steps based on clinical protocols Provide health education, medication assistance, and recommendations for self-care and follow-up appointments Conduct Transitional Care Management encounters for post-hospital patients, ensuring effective communication and care navigation Required qualifications Graduate of a School of Nursing; BSN preferred Current, active RN license with a compact license or application for the Nurse Licensure Compact Minimum of five years of experience as a registered nurse, with preference for emergency department or telephone triage experience Proficiency in using electronic medical records and basic computer skills Strong critical thinking, assessment skills, and the ability to communicate effectively in high-stress situations
