Activate Care is seeking a Registered Nurse to travel within Massachusetts to perform in-person health assessments for dually eligible members. This is assessment and documentation work, not ongoing care management.
The RN will complete UCA, FA, and Comprehensive Assessments, document findings in GuidingCare, and hand off to the health plan’s care management team. Must be licensed in MA, hold a valid driver’s license, and be able to work independently in members’ homes.
