In-Home PT: Independent, Patient-Centered Care

Southwoods Health is seeking a Physical Therapist to provide in-home, patient-centered care across Mahoning, Trumbull, and Columbiana Counties. You will lead evaluation and treatment in the home setting, with a focus on restoring function, reducing pain, and preventing disability after injury or surgery.

This full-time role requires licensure in Ohio, AHA BLS, and prior Home Health experience is preferred. Collaboration with a multidisciplinary team and supervision of support staff are key
