CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in their own homes. Reporting to the Clinical Manager, you will deliver personalized care focused on functional improvement and quality of life.
You will plan and administer prescribed PT treatment, assess patient progress, coordinate with a multidisciplinary team, and document evaluations and discharge summaries to support optimal outcomes.
