CenterWell Home Health is seeking a compassionate Physical Therapist to deliver personalized therapy in patients’ homes, report to the Clinical Manager, and support functional recovery and independence. You will assess patients, plan and administer prescribed PT treatments, supervise PTAs and aides, document progress, and coordinate with physicians and the care team to optimize outcomes.
Must hold a current unrestricted PT license and CPR certification; 40-hour work week; drivers license
