CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in their homes, reporting to the Clinical Manager.
You will plan and administer prescribed physical therapy, assess patients' progress, develop rehabilitation programs, and supervise PTAs and home health aides while keeping thorough documentation for physicians, the care team, patients and families.
