In-Home Physical Therapist — Impactful Care

Humana Inc. – CenterWell Home Health in Marlborough, MA is seeking a Home Health Physical Therapist to provide in-home therapy, assess patient needs, and develop rehabilitation programs aligned with physician plans. You will supervise PTAs and HHAs, document care, and report progress to the care team and family.

The role requires current PT license, CPR certification, and teamwork experience. One year of PT experience is preferred.
