CenterWell Home Health is seeking a Home Health Physical Therapist who will assess patients at their homes and deliver prescribed therapy to improve strength, mobility, and independence, following the physician’s Plan of Treatment.
You will document progress, coordinate with PTAs and home health aides, and report results to physicians, the Clinical Manager, patients and families. A current PT license and CPR certification are required.
