In-Home Nurse Practitioner or Physician Assistant Per Diem - Fort Wayne

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

Job Highlights

• The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and optional diagnostic tests such as diabetic eye exams and spirometry.
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Clinicians are not required to prescribe medicine, order lab tests, or alter current treatment regimens.

In This Role, You Will

• Bring your heart into every visit and join a national network of purpose-driven clinicians dedicated to improving health outcomes.
• Help visit millions of people nationwide, connecting them to the right care for their unique needs.
• Work with Signify Health as a clinician colleague and fulfill the requirements of the specified role.
• Be flexible to travel locally and to licensed states, as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).

A Note On Our Work Environment

As an in-home clinician, your primary workplace is the private residence of health plan members. You must be able to comfortably and safely perform duties in homes that may contain pets, secondhand smoke, and other airborne allergens or irritants.

Required & Preferred Qualifications (NPs and PAs)

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Pay Range

$37.66 - $81.11 per hour. This range represents the base hourly rate or annual full-time salary for the job grade within which this position falls. The actual base salary will depend on experience, education, geography and other relevant factors.
