Matrix Medical Network in the United States seeks experienced Nurse Practitioners to perform Comprehensive Health Assessments at patients’ homes or in skilled nursing facilities. Each visit can last up to one hour and focuses on health history, medication adherence, and overall well‑being.

You will work with physicians, case managers, social workers, and families, traveling up to 80–90% across states, to close care gaps and improve outcomes.
