In-Home Nurse Practitioner for Home Health Evaluations

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to perform in-home health evaluations for members in their own homes.

You will connect with members, review medications and medical history, perform a physical assessment, and arrange additional tests as needed. Travel locally within licensed states and occasional virtual visits are expected.
