UnitedHealth Group's Optum HouseCalls team is seeking an Advanced Practice Clinician (NP/PA) to conduct annual in-home health assessments for Medicare Advantage and other plan members. This non-prescribing, field-based role focuses on education, gap closure, and collaboration with primary care providers.
