In-Home Nurse Practitioner: Compassionate Field Visits

Signify Health is seeking an in-home clinician to perform medication and health history reviews, physical evaluations, and related diagnostic services in members’ homes. You will use a mobile workflow and must be comfortable with home environments and potential allergens.

The role includes part-time hours with hourly compensation plus incentives per visit, and malpractice coverage during Signify Health activities. Travel locally within licensed states is expected.
