In-Home NP/PA Clinician — Per Diem

Signify Health, part of CVS Health, seeks a Per Diem Clinician (NP or PA) to perform in-home health evaluations across Fort Collins area. Position is part-time with local travel, visits in members’ homes, and optional virtual visits.

You’ll use an iPad and a clinical workflow without prescribing or altering treatment unless required. The role requires active licenses, board certification, and the ability to cover Fort Collins MSA; multi-state licenses are a plus.
