In-Home NP/PA Clinician — Per Diem, Flexible Hours

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. Visits last about 45 minutes and include medical history review, physical evaluation, and optional tests as ordered by the health plan.

You’ll use an iPad and a streamlined workflow at the patient’s home. We value compassionate clinicians who connect with members in their homes, visit communities nationwide, and travel locally when needed.
