In-Home NP/PA Clinician — Per Diem & Flexible Visits

Signify Health is seeking a Per Diem Clinician (NP/PA) to perform in-home health evaluations, visiting members at their residences. The role is part-time with travel within licensed states and virtual visits as needed.

You will review medical history, perform evaluations, and provide Signify Health Diagnostic & Preventive Services where appropriate. Active unrestricted licenses and board certification are required, with multi-state licenses preferred.
